How to Become a Patient

Are you scheduling Pilonidal Surgery at The Sternberg Clinic? Follow these steps to become a patient.

Step 1: Fill out the Questionnaire

Tell us about your current health condition, your medical history, and your personal background to help your surgeon and our office staff better serve you through the pre-op and post-operation stages of your treatment at The Sternberg Clinic.

Step 2: Photograph your wound and send us photos

If you are traveling for surgery, help your surgeon better understand the location and condition of your Pilonidal Disease by photographing the wound area and emailing us photos. Please follow our specific guidelines for photographing this hard to reach area and submitting them to our office staff.

Step 3: Complete the forms and paperwork

Work with our staff to submit review and sign the required paperwork.  Contact our office staff with questions. Email all documents to us at info@thesternbergclinic.com
